APPOINTMENTS
Getting Started: We accept referrals from physicians, physician assistants and certified nurse practitioners. If you are interested in becoming a patient please contact the specific location during business hours or discuss your concerns with your doctor who can prescribe an evaluation. All therapy services require a current prescription.
After you receive a prescription from your doctor, please contact our office and we will schedule an appointment for an initial evaluation. A licensed therapist will perform a detailed examination expanding on your doctor's diagnosis. This evaluation includes several assessments such as Range Of Motion, strength, posture and balance. Your treatment plan may include modalities (ultrasound, electric stimulation), manual therapy, strengthening exercises, stretching and a tailored home exercise program.

Insurance Verification: We accept most insurances and our staff verifies each patient’s coverage before scheduling appointments. We perform an initial health screening and monitor each patient’s health throughout their treatment.
Appointments: We schedule therapy appointments according to patient and therapist availability. We book standing appointments and can often carry them over from week to week. We make every possible effort to schedule patients at times that are most convenient and offer early and late appointments as needed.

BILLING & INSURANCE
Flagship Outpatient Therapy accepts most private insurances, Medicare and Medicaid as well as other PPO/HMO plans.
It is each patient's responsibility to know and understand their insurance benefits. Our billing department will make every effort to obtain specific benefit information as it relates to the patient's therapy before treatment is initiated.
Due to the recent changes in healthcare, it is more important than ever for patients to call and verify their benefits with their insurance company.

HOME HEALTH
Flagship Rehabilitation provides full service on and off campus home health therapy management and staffing services. We will partner with your home health agency or with community-based home health providers that serve your campus. Residents will experience a personalized transition of care maintaining the same clinician through multiple Post-Acute Care settings.
Restorative
Flagship Rehabilitation offers Restorative Management Services to oversee your internal restorative nursing program, providing scheduling support and compliant licensed nurse documentation.
Section C & K
Flagship Rehabilitation will complete Section C (Cognition) and Section K (Nutrition and Swallowing) of the MDS to promote optimal and accurate coding and reimbursement under the Patient Driven Payment Model. This will help your community to capture relevant clinical indicators and provide compliant documentation under PDPM.

TELEHEALTH VIRTUAL SERVICES
1. Will Virtual Therapy benefit me as much as ‘hands on’ therapy?
Therapy provided via a virtual platform can still benefit you in many ways. Your therapist can review exercises to ensure that you are doing them correctly and safely, provide education on topics like pain management and activity modification to improve your function and many other beneficial interventions.
2. What if using virtual technology is challenging for me?
You will receive a Virtual Appointment Patient Packet prior to your first virtual visit that will guide you through the process and our staff will be more than happy to provide assistance as needed.
3. Can I use my phone or tablet for my appointment?
Yes you can use a smartphone, tablet, laptop or desktop for your appointment.
4. What if I don’t have a webcam?
That’s okay! If you don’t have a webcam Flagship can provide you with a device for your appointment.
5. Is my home environment well suited for a virtual therapy appointment?
Yes, Virtual therapy services can be safely and effectively completed in your home.
6. Will I get to see my therapist – or will I be working with someone else?
As with in-person visits, we try the best that we can to keep your therapist consistent.
7. Does my insurance cover virtual therapy?
Yes, Medicare is covering virtual therapy services during this public health emergency. Most other insurances are also covering these services during this time.
8. How long does a virtual appointment last?
Your virtual therapy appointment will depend on your needs and the therapy interventions that are being provided. On average- virtual appointments last about 30-45 minutes.
9. How do I make an appointment?
Just like any regular course of therapy, we will need a physician’s prescription to initiate virtual therapy services. Please contact Amber Carpen, PT Clinic Director with any questions and scheduling.
10. Can I switch to in-person services once it is safe?
Yes. These virtual services are being offered at this time so that you can continue to access essential therapy services safely in your apartment to minimize your exposure risk to COVID-19. You will be invited back to in-person services once it is deemed safe to do so.
